Diagnosis: Type 2 diabetes. A lifestyle adjustment is often necessary to help manage this disease, and that may seem overwhelming. However, by taking small steps, people with type 2 diabetes can gradually make the changes needed to better manage their disease. The Game Plan, a component of the patient education program Blood Sugar Basics—developed by the American College of Endocrinology with support from Merck—breaks down diabetes management into four "goals" to be tackled one at a time. Each "goal" also includes helpful tools and resources.

Goal #2 - Enter the Nutrition Zone: develop meal plans
Goal #3 - Get in the Game: get active
Goal #4 - Check the Scoreboard: assess your progress

The Game Plan's "coaches" are Mike Golic, co-host of ESPN's Mike & Mike in the morning and former NFL star who has type 2 diabetes; Farhad Zangeneh, M.D. FACT, FACE, a clinical endocrinologist; and Samantha Heller, MS, RD, CDN, a registered dietician and exercise physiologist with expertise in diabetes.
