To be eligible for the program, you must not have any prescription drug insurance or benefits through: private insurance, employer-sponsored insurance, HMOs, Medicaid, Medicare prescription coverage, State Pharmacy Assistance Programs or veterans assistance. You also must reside in the United States and have a treating physician licensed in the United States. (Note: The Merck Prescription Discount Program is not insurance.)

• Enroll online, or
• Request an enrollment form be mailed to you, or
•  Print enrollment form and mail to:  Merck Prescription Discount Program, PO Box 369, Horsham, PA 19044-9945, or
• Mail in the simple enrollment form included in the back of patient brochures available at pharmacies and physician offices.
